Case story: design-flaw discovery

A SCADA Panel Replica That Caught a Sister-Site Incident Before It Happened

On an investigation in Northern Ireland, our chartered officer built a full-size replica of the SCADA control panel involved in the incident. Working through the panel in the lab, the officer identified a design flaw in the switchgear layout that had nothing to do with the operator on shift the day of the event.

The same panel design was in service at a sister site in Ireland. We escalated the finding into a corrective action that crossed both locations, and the design change was implemented before a second incident could occur. When should we call you in after an incident?

As early as you can. Site evidence degrades within hours of an event, witness recollection drifts within days, and statutory reporting clocks (UK RIDDOR, Irish HSA notifications) start immediately. We accept first-call notifications 24 hours a day and have a chartered investigator on site or on a structured remote brief within 48 hours, guaranteed in writing. Calling us in early protects the integrity of the root-cause analysis and gives you a defensible chain of custody for evidence and interviews.

How do RIDDOR and Irish HSA reporting timelines work?

Specified injuries, dangerous occurrences, and over-7-day absences must be reported to the UK HSE under RIDDOR 2013 within set windows (10 days for over-7-day injuries, immediately for fatalities and specified injuries). Equivalent obligations apply under Irish General Application Regulation reporting to the HSA. We prepare the statutory report on your behalf against the evidence we gather on site, line it up with your insurer notifications, and submit on the regulator portal. The same chartered advisor stays with the file through any follow-up correspondence.

How independent is the investigator from our project team?

Fully independent. Our investigator is engaged by the client organisation but reports findings without any reporting line into the operational team that managed the incident. The investigator is a CMIOSH-chartered professional and was not part of the project being investigated. This is the independence that the regulator, your insurer, and any subsequent inquiry expect to see in the file. We refuse engagements where the brief would compromise that independence.

Who owns the investigation report and the corrective actions?

You do, fully. The report, the evidence pack, the root-cause analysis, and the corrective action register are your property from the moment they are issued. All documentation is specific to each project and never pulled from a template library. We support the close-out of the corrective actions on a retained basis if you want continuity into the next audit cycle, but ownership stays with your organisation throughout.

What kind of corrective actions actually get implemented?

The corrective action register is written so each item has a named owner, a target date, a verification method, and a link back to the root cause it addresses. Typical actions include revised RAMS and Method Statements, updated training matrices, design or guarding changes, supervisor briefings, and amendments to the Construction Safety Plan or Preliminary Safety Plan. We feed the register straight into the next safety policy review and the next site audit, so the close-out is visible to the regulator and to your senior team.
